Developmental Disabilities Registration Program

To better serve those community members who may suffer from developmental disabilities the Lisbon Police Department has created a registration program. The below attachment will bring any family member, or caregiver, to a form where you can register individuals with developmental disabilities who either: reside, receive services, attend school, or work in the Town of Lisbon.

The registration form collects information about a person's tendencies, triggers, and any other information that the caregiver thinks the police department should be aware of. The information will be secure here at the police department, with easy access to Lisbon's first responders should any assistance become necessary.

To register an individual for the program, a family member or caregiver could download this registration form, or come into the police department dispatch window and request one. Once completed the form should be dropped off here at the police department, with a photograph of the registrant.
